Documents Required
In accordance with AFI 36-3026(I), identification cards for members of the Uniformed Services, their family members and other eligible personnel, joint regulation and/or local policy, the following documentation is required for issuance of military identification cards and/or DEERS enrollments.

DUE TO THE PRIVACY ACT, THE SPONSOR/SERVICE MEMBER MUST BE PRESENT TO AUTHORIZE ALL DEERS/ID CARD ACCESS/TRANSACTIONS OR A DEPENDENT MUST PRESENT A CURRENT GENERAL OR SPECIAL (ID CARD/DEERS) POWER OF ATTORNEY OR DD1172 AND A CURRENT PHOTO ID (NOT A PASSPORT). FAMILY MEMBERS MUST BE ENROLLED IN DEERS BEFORE AN ID CARD CAN BE ISSUED.

DEERS Enrollment
What do I need to enroll my spouse in DEERS? Original or certified copy of the marriage certificate, social security card and one form of original photo ID. If either sponsor or spouse was previously married, ALL final divorce decrees and/or death certificates are required. 

What do I need to enroll my child/stepchild in DEERS? SPONSOR needs to present the original or a certified birth certificate and social security card. Single Male Soldier- A court order for child support or court issued paternity determination or order of affiliation or acknowledgement of paternity, etc. Documentation must show where the state has accepted you as being the father and the original or certified copy of the birth certificate. If it is the sponsors stepchild, original or certified copies of the marriage license of the parents, birth certificate and social security number. 
What if my child is over the age of 21 years of age? A dependent over the age of 21 years of age can be eligible for DEERS under the full time student status, INCAP or TYA. If the dependent is a full time student they will require a letter from an accredited college stating enrollment as a full-time student with the anticipated graduation date. Letter can be no more than 90 days old and must be signed by the college registrar. If they are an incapacitated child over the age of 21 DFAS approval is required from the service member’s parent service. A dependent over the age of 21 can also be enrolled in DEERS under the Tricare Young Adult Health Program and the member needs to contact TRICARE for more information (1-877-TRICARE).

What if my child is adopted? SPONSOR needs to present the final Adoption Decree and original or certified copy of the birth certificate.

What if my dependent is a ward? Sponsor will need to bring the court documents signed by a JUDGE placing the child in your legal custody for 12 consecutive months and COURT DOCUMENTS MUST REFLECT THE CHILD IS IN THE CUSTODY OF THE SERVICE MEMBER

ID Card Issuance for Service Member
My CAC card is expiring what do I need to renew it?
You will require your expiring CAC card and another form of valid ID *PLEASE NOTE CAC CARDS ARE ELIGBLE FOR RENEWAL 30 DAYS PRIOR TO EXPIRATION

What if I lost my CAC card? If lost/stolen you will need a police report (per the DTM 08-003 Attachment 3 Section 8B) and two forms of valid ID

My CAC card is not working/damaged what do I need to replace it? You will need the old CAC card and another form of valid ID. *PLEASE NOTE IF YOU ARE AN E-5 AND BELOW (NAVY) YOU WILL NEED A REQUEST CHIT BOTTOM LINED BY E-7 OR ABOVE IN YOUR CHAIN OF COMMAND IN ADDITON TO YOUR CAC AND FORM OF ID

ID Card Issuance for Dependents

What does my spouse need to renew his/her ID card?
They will require two forms of unexpired ID. If your spouse is approaching the age of 65 (within 90 days) they will also need to bring in their MEDICARE card.

What does my full time student need to renew their ID card?
They will require a letter from their school verifying full time student status, they are working towards an associate’s degree or higher, with an expiration date from the school registrar and two forms of valid ID.

What if my child is enrolled in TRICARE YOUNG ADULT HEALTH?
After they have received the welcome letter they will also require two forms of valid ID.

What if my child is an INCAP child over the age of 21?
They will require the approval letter from DFAS and two forms of valid ID

At what age can my dependent (CHILD) obtain an ID card?
ID cards are issued at the age of ten. If a sponsor is mil to mil or a single parent then the child is eligible for an ID card at any age. A need a copy of their birth certificate should be brought in along with the child.
