Hours of Operation

7:30 a.m. - 4:00 p.m. (Tuesday – Saturday) Walk in cut off time is 1500

Closed (Sunday - Monday)

Remember to bring two forms of identification, such as a current Drivers License or identification card issued by a state or local government agency.

If sponsor is not present, you must provide either a Power of Attorney or a DD Form 1172 which must be verified and issued by DEERS and sponsor’s signature must be notarized.

Avoid longer wait times by making an appointment online by logging onto https://rapids-appointments.dmdc.osd.mil.

Appointments are available to active duty, guard and reserve, DoD civilians and contractors, retirees, and disabled Veterans (DAV) members.  Dependents of active duty, reserve, retirees and DAV members are also able to make appointments online.

Mail-In Request for I.D. Cards

This is not an official document! This is simply a guide.

For persons that are incapacitated and need to renew their I.D. by mail the following documents (a, b, c, d, and e) must be presented:

a.) 5x7 or 8x10 photograph head shot (front view shoulders to the top of the head) On the back it must have:

  • Full Name
  • Height
  • Weight
  • Eye Color
  • Hair Color
  • Notarized by a notary public


b.) Copy of current Drivers License or Identification card issued by a State or Local Government.

c.) Old I.D. card.

d.) Letter from the doctor stating you are incapacitated and unable to travel.

e.) If sponsor is still alive, a completed and signed DD Form 1172 signed and notarized.
