Emergency treatment and medical evacuation (MEDEVAC) overseas can easily cost tens of thousands of dollars for non-SOFA personnel. If you are planning to host friends or family members who are not categorized as legal dependents, for any period of time, ensure they purchase traveler's insurance, specific to the location of travel (i.e. Japan) prior to travel.
Non-SOFA status civilians are NOT authorized to receive care at military treatment facilities (MTF) nor at the local Japanese hospitals. If a civilian does not have insurance and needs emergency care they will personally incur all medical and transport costs (out-of-pocket expenses).
