Applying for Housing

An advanced housing application, although encouraged, is not necessary as it does not allow for a Service Member's name to be placed on a waiting list in advance.

Applicants awaiting housing assignment are responsible for verifying and updating their status on the waiting list a minimum of once every month.  Failure to do so may result in the applicant's name being removed from the waiting list.  Should this occur, requests for placement back on the waiting list will result in an applicant's control date being the date the applicant reapplies for housing.

When an applicant's name is reached on the waiting list, he or she will be contacted by a representative at the Public-Private Venture (PPV) neighborhood management office and advised as to the date a PPV home will be available for occupancy.

Assignment of a Service Member to PPV housing normally will not be made when the anticipated tour of duty at Naval Station Great Lakes is less than 6 months.

Acceptance of PPV housing requires that Service Members arrange for an allotment of their basic allowance for housing (BAH) to the PPV partner.  For questions regarding the BAH allotment process, contact the Naval Station Great Lakes PPV neighborhood management office at (847) 689-4312.


APPLICATION PROCESS

Applying for PPV housing is a simple three step process:

Step 1 – Fill Out Application
Complete, sign and date a DD Form 1746 (Application for Assignment to Housing) and Sexual Offender Notification Addendum, which can be obtained at your local installation Navy Housing Service Center (HSC) or by clicking on the links listed below.

Step 2 – Gather Your Records
Fax, mail or hand deliver to the Navy HSC a completed, signed and dated DD Form 1746 and Sexual Offender Notification Addendum.  Also include, a copy of your permanent change of station (PCS) orders, Page 2 (Dependency Data Record) or DEERS enrollment form, and Detachment Endorsement and/or release document that shows the day, month and year released from your duty station prior to execution of your new PCS transfer orders.  Please Note:  The date of detachment from the losing command is used as your control date for placement on the waiting list upon arrival at the Great Lakes NavyHSC 30 days prior to reporting to your gaining PCS command at Naval Station, Great Lakes.  Service Members who report to the Navy HSC 30 days after reporting to their respective gaining PCS command will be placed on the waiting list with a control date effective the day of arrival at the Navy HSC.

Additionally, if the Service Member will be unavailable at the time of lease signing for PPV housing, a current Special Power of Attorney document will be required for the spouse to sign and accept a PPV home on behalf of the Service Member.

Step 3 – Report to Housing
Please ensure that you arrive at the Navy HSC within 30 days of reporting to your gaining PCS command to obtain the most up-to-date information regarding the waiting list and PPV housing assignment availability.
