Commanding Officer's Suggestion Box

PRIVACY ACT STATEMENT

AUTHORITY: 10 U.S.C. 5013, Secretary of the Navy; 10 U.S.C. 5041

PURPOSE: To provide service members, DoD employees, retirees and family members an equal opportunity to share their ideas and concerns with the installation Commanding Officer.

ROUTINE USES: In addition to those disclosures generally permitted under 5 U.S.C. 552a(b) of the Privacy Act, these records or information contained therein may specifically be disclosed outside the DoD as a routine use pursuant to 5 U.S.C. 552a(b)(3) as follows: The DoD 'Blanket Routine Uses' that appear at the beginning of the Navy's compilation of systems notices apply to this system.

DISCLOSURE: Voluntary.  However, failure to provide the requested information may cause lengthy delays and/or the inability to complete the request.

As the Commanding Officer of Naval Magazine Indian Island, I appreciate hearing your feedback, comments, questions and concerns. This virtual CO’s Suggestion Box is an avenue for any member of the Indian Island team to reach out to me confidentially, and if you so choose, anonymously.  

I am always interested in receiving input with new ideas about how we can improve our operations and suggestions on how I can better support you and the team overall.

If you or someone you know is in crisis and having suicidal thoughts, please know there are resources available to you.  Remember, ACT - Ask, Care, Treatment!

Sailors can call the Military Crisis Line (call 800-273- TALK and Press 1 or text 838255) for assistance.  Sailors are encourage to help a fellow shipmate in need by getting them in contact with the nearest chaplain, provider or leader, or calling 911 if danger is imminent.  Additional information is available at www.suicide.navy.mil.

Civilian employees can access assistance from the Department of the Navy Civilian Assistance Program (DONCEAP) by dialing 1-844-DONCEAP (1-844-366-2327).
